Police Record Check Forms

All coaches, assistant coaches and managers are required to provide a Police Record Check to OMHA. Please see the below links for the document you will need to provide to your local law enforcement agency where you reside (Okotoks RCMP or Calgary Police Service):

You must present two forms of identification upon application. There will be a $5.00 fee for the initial processing, and a $10.00 fee if fingerprinting is required. Payment must be in cash.

Once you have received your police record check please email a copy to Lindsay Graw at admin@okotokshockey.com.

The Okotoks Minor Hockey Association (OMHA) administers a minor hockey program for residents in Foothills County, including the Town of Okotoks. The current OMHA hockey program exists for players ages 5 years through 18 years, and consists of High Performance Level (AAA, AA), Community Level (A,B and C)

OMHA has a working partnership with 3 organizations to administer the AAA and AA levels of hockey - the OOAA (Okotoks Oilers Athletic Association), FMHA (Foothills Bisons Hockey Association) and the Foothills AAA Hockey Association.
